Developing Community Skills Pathfinder
The problems of unnecessary hospital admissions and protracted hospital stays are a result of poor collaboration of health and social care. A fundamental element of this project was to equip nurses in care/nursing homes with the skills to be able to work effectively with their peers in health so there are seamless care pathways. The project also pro-actively promoted better communication between care workers and healthcare assistants.
The project was incredibly successful and resulted in £72,000 savings, improved quality of care and increased staff morale.
